Returning to Germany: Health Insurance, Status and What You Should Know Beforehand

Returning is not a rare edge case. According to an analysis based on destatis figures, roughly 742 Germans emigrated to Paraguay in 2025, and about 508 returned; over the period from 2014 to 2023, roughly 6,208 departures stand against about 3,714 returnees. So if you move to Paraguay, you should think about the way back from the very start.

The biggest long-term risk is health insurance. Rejoining the statutory health insurance (GKV) after returning from a non-EU country like Paraguay is not automatic. Without a so-called Anwartschaftsversicherung (dormant-cover insurance), you usually only have a right to join if you take up employment subject to social insurance again (reported: within roughly two months of your return). For people aged 55 and over, rejoining the GKV becomes especially difficult.

Why rejoining the GKV is not automatic

When you leave Germany, your compulsory membership in the statutory health insurance ends as soon as you move your residence or habitual abode abroad (section 190 (13) SGB V). The deregistration certificate is the usual proof of this. In Paraguay the German GKV pays out nothing anyway, because there is no social insurance agreement between Germany and Paraguay; from day one you need an international or local health insurance there.

The decisive point for returning: anyone who simply cancels the GKV on departure has no automatic entitlement to be readmitted on return. According to reports, without provision you usually only have a right to join if you take up employment subject to social insurance in Germany again, and do so promptly after your return (reported: within roughly two months). Anyone who no longer works as an employee, for example as a pensioner, self-employed person or private individual, often does not have this route.

Legal position vs. experience value

Legal position: compulsory GKV membership ends when you move your residence abroad (section 190 SGB V), and it does not pay out in Paraguay (no social insurance agreement). Experience value: the exact two-month window, how it is handled from age 55 and the right to join depend on the individual statutes of your insurer. That is not a fixed figure for everyone. From 55 it gets especially tight

The biggest hurdle hits older returnees. According to reports, people aged 55 and over run into particular barriers when rejoining the GKV. The background: anyone who has been outside the GKV for a longer period and, when rejoining, no longer meets certain prior-insurance periods may find themselves neither in the statutory system nor easily in the private one.

That is exactly why the question of health insurance is the most important topic for emigrants over 50 to clarify before departure, not only on return. Anyone who moves to Paraguay at 60 and wants to return at 68 decides, with the choice made on departure, whether the way back into the GKV is even still open.

The Anwartschaftsversicherung as provision

The Anwartschaftsversicherung is the instrument with which you secure your right to rejoin without paying the full contribution during your time in Paraguay. Both the statutory and the private health insurance offer it. You pay a reduced contribution, receive no benefits during your time abroad in return, but obtain the right to rejoin on your return without new admission conditions.

In the GKV, one insurer cites an Anwartschaft contribution of roughly 84.56 EUR per month as an example for 2026 (Bosch BKK); the exact amount depends on the statutes of the respective insurer and varies. In the private health insurance a distinction is made between the small Anwartschaft (rejoining without a new health assessment) and the large Anwartschaft (which additionally preserves the ageing reserves). Whether and how an Anwartschaft is offered depends on the statutes of the respective insurer.

The Anwartschaft covers nothing abroad

Important: an Anwartschaftsversicherung only secures your right to rejoin in Germany. It pays for no treatment in Paraguay. There you always need an additional international or local health insurance. The Anwartschaft plus the international policy are two separate contracts that run in parallel.

Your status in Paraguay stays in place predictably

Flying back to Germany does not automatically mean your Paraguayan residency lapses. The status remains in place as long as you keep to the presence rules. For the path to permanent residency, since 1 January 2026 the authority checks your entries and exits: if you have been outside Paraguay for more than 365 consecutive days since your temporary residency, the application can be rejected. A short visit within twelve months is usually enough to keep the clock intact.

Re-registration, pension and child benefit

When you return to Germany, you register your residence again. As soon as you live and work in Germany or once again have your habitual abode there, the German systems generally apply again, potentially including the way back into the GKV via employment subject to social insurance.

The German statutory old-age pension is paid worldwide, so also during your time in Paraguay, and its basic entitlement does not change through a later return to Germany. As long as you live abroad, the Deutsche Rentenversicherung (DRV) usually requires an annual life certificate (Lebensbescheinigung) there. Child benefit, on the other hand, requires a residence or habitual abode in Germany (section 62 EStG); it ends when you move to Paraguay, and on your return to Germany it can be applied for again under the general conditions.
